It does look like there is some mild elongation in his molar roots. I have seen much worse, but you should always treat the patient, not the x-ray. I do not see any bone loss at the base of the molar roots either. When there is bone loss, usually the pain is so bad that even filing the teeth down does not stop the symptoms.
I don't want you to give up on him, but I am worried because he did not respond to the treatment that there may be something that we are not seeing on those x-rays.
That is definitely the best place to go if you have a chin near Chicago!! I usually see improvement in clinical signs no later than 1 week after the grinding, so hopefully that will help you judge his improvement. Just talk to the vet and see what happens - I think it is worth one more shot if they agree to try filing all of the molar crowns down to below the gum line. Good luck!
