Ok so I think my brain is completely fried and I'm really getting frustrated working on my careplan.

I have to have 3 nursing diagnosis, I've got 2 acute pain and impaired gas exchange.

My patient was admitted with a perforated bowel following a colonoscopy. I have come up with a few nursing diagnosis that I could use BUT the kicker is I have to have 3 realistic, measureable goals per diagnosis and I can't come up with enough per diagnosis that I come up with.

My patient had an NG tube so I was thinking imbalanced nutrition less than body requirements b/c he is NPO or fluid volume deficit but can't come up with the goals

I also thought about ineffective gastrointestinal perfussion but definitely couldn't come up with measurable goals here because his labs weren't that bad.

I could use infection because his WBC count is up but other than watching his WBC count daily I couldn't come up with any other goals since the infection site is inside the body and not out. I've been going through my nursing diagnosis manual for a while now and I'm about to give up so I need some reinforcements!:cry3:

I know there are several people out there that are in nursing school or are nurses so if you have any ideas that could help me I would greatly appreciate it!!
 
Ahh, the nursing diagnosis. My husband is just a year out of school and I'd ask him to help you, but we were just chatting over the weekend about how stupid nursing diagnoses are and how he doesn't remember any of them. NO ONE uses them in the real world of practicing medicine. It's a shame they waste so much of your time in school learning something this useless.
 
I would do risk for fluid volume deficit r/t (GI losses, no intake,) Goals could be pt will state risk factors, pt will be able to maintain NG tube feedings, pt will have intake and output between these ranges (not sure of age, weight, so I can't figure that up). Imbalance Nutrition... goals could be pt will maintain weight, pt will have lab values that reflect proper nutritional status, pt will take supplemental nutrition. Infection... goals could be pt will have WBC levels within normal range, pt will be afebrile, pt will have state interventions to reduce infection. If you need nursing interventions just let me know... I have like 3 books and 1 program on my palm for nursing diagnosis... I'm in my last year of nursing school, so I definitely feel for what you're going through right now! Good Luck :) Just PM me if you need anything more
