What questions to ask during screening and caresheet for new owners?

In another thread about selling one of my chinchillas screening potential customers was mentioned. Is there a list somewhere of what screening questions to ask? If not, then could one be compiled? I know there are the obvious questions such as what kind of cage, temperature of room, but I am the kind of person whose mind goes blank during interviews and a list would be very helpful.
It would also be helpful to have a care sheet or booklet that could be provided when chinchillas are purchased. Does anyone have one they could suggest or share please?

I made it for a couple of friends who run rescues to use, to help educate new chin parents.
